Summary:The NEPQR outpatient therapist will be a generalist clinician, with established clinical and outreach experience serving diverse and underserved populations. The individual will be an integral part of a grant-funded interdisciplinary team based out of the GVSU Family Health Center (FHC) and bringing behavioral health services to low-income housing residents of Dwelling Place (DP) properties in the Grand Rapids area as well as members of the GVSU community.
This position has the opportunity for a hybrid work arrangement.
Required Qualifications and Education:
- Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW).
- Licensed to provide counseling in the State of Michigan
- Master’s degree in social work from an institution accredited by the Council on Social Work Education (CSWE).
Preferred Qualifications and Education:
- Previous experience providing care management services to individuals with persistent and serious mental illnesses.
- Experience working in a care setting using an interdisciplinary team approach.
- Experience working with individuals with substance abuse challenges.
- A minimum of two years of experience providing psychotherapy services to vulnerable populations.
Responsibilities – Essential Functions:
The outpatient therapist will be a valuable member of an interdisciplinary team which will be based at the GVSU Family Health Center (FHC). They may deploy regularly to three DP housing sites: Herkimer Apartments, Commerce Courtyard Apartments, and Verne Barry Place. They will be responsible for the implementation of grant expectations which include but are not limited to:
- Collaboration with the mental health team to provide mental health and care management services to DP residents and patients of the FHC.
- Bringing unique perspectives, along with the Psychiatric Mental Health Nurse Practitioner (PMHNP), for evidence-based interventions and plans of care for each patient.
- Collaboration with Dwelling Place staff who work with the residents on a daily basis to develop trusting relationships with residents.
- Coordinating care management and behavioral health interventions using clinical judgement and expertise.
- Participation in the following meetings: NEPQR-BHI Advisory Board, focus groups, Corporation for Supportive Housing (CSH) Frequent Users Systems Engagement (FUSE) initiative, NEPQR Behavioral Health Integration (BHI) Team Meetings, site visits with Project Manager and/or Project Director, and team huddles.
- Enhancement of the existing team of primary care providers in an effort to increase the FHC’s capacity to support patients with behavioral health challenges.
- Working with the grant team to provide valuable feedback to the design and implementation of the program.
Responsibilities – Non-Essential Functions:
- Independent judgement and functioning with accountability for own actions, accuracy of work, and supervision of actions and accuracy of work of others.
- Effective communication and coordination between the Principle Investigator, Project Manager, GVSU FHC staff, community members, students, and other faculty/staff.
Salary: As outlined in HRSA NEPQR-BHI Grant. This is a full-time, 0.8 FTE position (32 hours per week)
